En06-A@1#prop:EB
A quantum-to-classical channel followed by a decoder whose only nontrivial input is that record is entanglement breaking. It cannot recover every unknown quantum state with its reference.
Proof. The composite has form
With a reference, its output is a sum of positive product operators, hence separable. The identity channel preserves a maximally entangled input, so the channels cannot agree.
