Geometry, bundle, and non-global-hyperbolicity

This reproduces the relevant geometric obstruction of I-B-G :nonGH without importing a global Cauchy theorem. A self-adjoint extension supplies the evolution used here; it does not make the regular slices Cauchy surfaces for all causal curves.
