Tomographic setting and measurement contract

A failure flag can be converted to a predeclared replacement output before scoring. If failures are instead retained as extra output states, the output dimension and tomography must be enlarged. Unsupported calibration or transfer hypotheses make the deployment verdict inconclusive.
