Model specification: Complete attempted-run scoring

Fix one finite code, its block length, its message distribution, and its decoder. Repeat it on independent preparations. Each attempted run receives a predeclared score in $[0,1]$; failure events remain in the record and receive their declared scores. No success-only postselection or unaccounted stopping rule is substituted.
