The paper supplies:

  1. an entrywise encoding matrix and a gate sequence;

  2. exact access results for all boundary subsets;

  3. matrix-defined accessible decoders for every authorized set, and a gate-level two-slot streaming decoder for a named triple;

  4. a complete channel and register ledger;

  5. simultaneous finite-data certification and its acceptance guarantee;

  6. certificate-specific feasibility calculations, exact binomial inversion, and justified structure-assisted conversion;

  7. an analytically solved flagged-erasure benchmark;

  8. a normalized two-pentagon network and a finite-factor subsystem certificate.

No validation data were supplied. Consequently no measured reconstruction map, numerical acceptance study, or hardware result can honestly be reported. The decision table in Section sec:reporting is a reporting interface, not an invented dataset.
