A failure-retaining amplitude instrument

A finite realization prepares an independent binary failure flag, destructively reads it, and consumes the signal in either branch. Coarse-graining the two failure outcomes is a channel specification. If a larger accessible apparatus exports those outcomes, that larger boundary must be compared separately.
