p22: evanescent components (EV-M2), evanescent counterterms (EV-CT) and the runtime d guard

. The tools/ and notes prefix exemptions are removed. Every changed, added or removed file relative to the p21 zip must be listed in DISCLOSED_REVISIONS_P22. The only exceptions are the generated reference values and the pinned archival/p21 records. Earlier-layer gates accept the registered p22 revisions only through contract_registry.
