Review fixes G1–G4

Scope of revision p17: the hh evanescent poles and the ch slot › Review fixes G1–G4 · lines 703–741

G2: dred.parse_d (locals ${d :$ opaque $d}$ ) and dred.assert_opaque_d close the plain-Symbol('d') hazard of re-parsed strings (disclosed contract revision; no value affected). G1/G3: the guard docstring and the disclosure now state exactly what is refused at runtime and what is excluded by the extended static AST scan: replace with any function query, sympify/S/parse_expr of non-sympy data without the guarded locals, builtin eval/exec, srepr round-trips, .func(*args) rebuilds, Poly(..).eval, lambdify. The scan is a pattern check, not a proof. G4: the change log lists the p17 reference-values file as added.
